TEAM VAN SCHEDULE IS AS FOLLOWS:
LEAVE: Saturday, 5:00am-NO BREAKFAST STOP
RETURN: Saturday, about 10:30pm
ENTRY FEE: Tournament entry fee is $35 (no checks, Chanbara $15 extra, $5 discount with USKA membership card)
All Yellow Stripe students and above are welcome and encouraged to compete. Families are welcome to meet us or follow us there. Individual students wishing to ride in the team van need to sign up by the Thursday before. We chip in $10 each when riding in the team van.

It will take a full TEAM effort if we wish to gain the State Championship team title as we did in 2000, we missed 1st place team title last year by 35 points (an individual 1st = 25, 2nd = 20, & 3rd = 15 team points). We would like for as many of our students to compete as possible, especially our Yellow Stripe and Yellow belt students, to give us our best chance. Also don't miss the after tournament swimming party at the Holiday Inn Holidome.

Directions: From I-20 west bound, take a left at the 3rd Ruston exit (La Tech), then take a left after the car wash.
RETURN: Should get back to Mandeville about Noon, Sunday.

In submitting said application, the undersigned hereby gives permission for the contestant to participate in said 2003 United States Karate Alliance Louisiana State Championships to be held at Ruston High School, Ruston, Louisiana on October 25, 2003. Acting for myself, or as the parent or legal guardian for the contestant, I do hereby assume all risk of injury to said contestant while participating in and traveling both to and from said United States Karate Alliance Louisiana State Championships, and acting for myself, my heirs, personal representatives and assigns, do hereby release the tournament promoters, United States Karate Alliance, and associations, the officers, agents, employees and representatives of said organizations, Ruston High School and any and all competitors and fellow participants, individually and collectively, from any liability (now and forever). As a parent or legal guardian, I agree to indemnify any such organization for any loss or expense by reason of any claim by child or ward.
